PHOENIX AGED CARE is an Approved Provider of My Aged Care Home Care Packages under the Aged Care Act. This means we can offer high-quality, government-funded home care services for levels 1 to 4.

Home Care Packages are an Australian Government-funded program designed to help older Australians who wish to continue living independently in their own homes rather than moving to a nursing home or assisted living facility.

The amount of government assistance you or your loved one receives depends on your assessed needs and personal circumstances, as determined by the Department of Human Services. The subsidy increases with each level according to your specific situation.
In some cases, you may need to contribute more towards your home care fees. If you are required to pay an income-tested care fee, the government subsidy will be reduced by that amount. For example, if your Home Care Package is assessed at $20,000 and your income-tested care fee is $5,000, your subsidy will be $15,000 ($20,000 - $5,000 = $15,000).
The subsidy is paid directly to your chosen approved provider, such as PHOENIX AGED CARE.
Home Care Packages are intended for older adults (aged 65+ or 50+ for Aboriginal) and those living with a disability who need support to remain in their own homes. They are not designed for those requiring entry-level, ongoing, temporary, or short-term care.
To determine your eligibility for a Home Care Package, contact us. They will refer you to the Aged Care Assessment Team (ACAT) or ACAS (for VIC) to arrange an in-home assessment of your needs and determine the appropriate Home Care Package level.
Approved Providers are private or public businesses that meet the Department of Health’s standards for delivering high-quality home care services to eligible recipients. As a recipient, you can choose your provider and the types of services you receive.
Approved Providers must deliver the services outlined in your care agreement. Be aware that some providers may charge additional fees for administration, case management, subscriptions, or other services. Ensure all costs are disclosed, not just the daily fees.
